I hereby acknowledge that I am willingly participating in a paid or unpaid session, class, workshop or program with Aubyn Philabaum Acting Studio, LLC, to include but not limited to, services such as acting coaching or career consulting, about any topic whose nature may be career, personal, or any other topic of discussion, teaching or counseling.

I understand that the above does not substitute for or replace legal, therapeutic, or any clinical advice and that it is my responsibility to secure such advisement.

I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from any coaching, teaching, consulting, program or guidance.

I understand that results are individual and may vary. I hold Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC, and all teachers and workshop facilitators who work and teach at the Aubyn Philabaum Acting Studio, LLC, harmless from any possible effects that may cause physical or emotional discomfort.

I do here and forever discharge and hereby hold harmless Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC, and her respective agents, heirs, assigns, contractors, and employees to any and all claims, demands, damages, rights of action or causes of action, past, present or future, arising out of or connected with my participation in a session or purchase of any program without limitation.

I further acknowledge I am granted permission to any and all final edited video footage from my private one-on-one coaching sessions with Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC. I acknowledge that should I desire the raw footage from a session, I must inform Aubyn Philabaum of this by the end of session and no later. I acknowledge that raw footage files are deleted by the end of each editing session and are unable to be retrieved.

I understand that all material is owned by Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC. This includes but is not limited to all files taped or recorded during a coaching session, class, workshop, or consultation with Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC and her respective agents, heirs, assigns, contractors, and employees.

Any unauthorized use, sharing, broadcast or release, personally or publicly in any form of a taped session or of any written material distributed by Aubyn Philabaum, Owner/Teacher of Aubyn Philabaum Acting Studio, LLC, is strictly prohibited, and would be considered an illegal act punishable by law.

I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THIS WAIVER AND RELEASE, AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY.

BY CHECKING THE BOX BELOW I WAIVE ANY RIGHT I OR MY SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST AUBYN PHILABAUM, OWNER/TEACHER OF AUBYN PHILABAUM ACTING STUDIO, LLC, AND ALL TEACHERS AND WORKSHOP FACILITATORS WHO WORK AND TEACH AT THE AUBYN PHILABAUM ACTING STUDIO, LLC, FOR NEGLIGENCE OR THAT OF ITS AGENTS OR CONTRACTORS.
